1. Original biopsy paper, along with glass slides and paraffin blocks if further testing is required. 2. Original X-rays, USG, CT, MRI, or PET-CT scan films and printed reports. 3. Recent complete blood count (CBC), kidney function, and liver function reports. 4. Summary of ongoing treatments for hypertension, diabetes, or cardiac conditions.
